This is the badge record that I use for my Junior troop. I incorporated a lot of ideas from other forms to come up with one easy to use form that I can live with. Hope this makes your record keeping a little easier!  Sorry, I don't know how to convert this to ASCII and also keep the spreadsheet intact.
